• Assists the RN in performance a comprehensive assessment and develops a specific plan of care to maximize treatment, reduce paid, reduce symptoms and discomfort, education and compliance of the patient in cooperation with the ordering physician.


• Assists the RN in developing patient care goals that are realistically achievable and aimed at meeting patient needs for comfort, elimination of paid and reduction of symptoms.  Involves the patient and family in planning goals, end-of-life care and educate to process for dealing with issues of ethical concern.


• Implement appropriate nursing care plan and provides nursing care in accordance with the agency policy, proper nursing procedure and the PA Nurse Practice Act and as ordered by the physician.


• Participates in weekly case conference meeting, completes interdisciplinary team notes timely and communicates any pertinent findings to the RN and other members of the care team.


• Assists RN to re-evaluate the plan of care for control of pain and assessment of the medication treatment plan in cooperation with the physician.


• Instructs and supervises home health aides and family members when appropriate and assists RN in coordination of total residential care.

• Graduate of an accredited school


• Current license in good standing to practice in the Commonwealth of Pennsylvania


• Minimum of one year experience in hospice, nursing home or medical /surgical setting


• Maintains clinical and professional competence


• CPR certified


• Possesses the ability to follow oral and written instructions


• Possesses the ability to communicate effectively with all members of the care team and families


• Possesses good organizational skills.

The Licensed Practical Nurse under the director of a licensed Registered Nurse provides nursing services to patients as ordered by the physician.  The nurse is proficient in assessing the patient’s condition and assists the Registered Nurse in developing a plan of care to improve, monitor, and document the course of treatment prescribed by the physician for the patient’s diagnosis.  The Licensed Practical Nurse assists the registered nurse in the care of the patients to reduce symptoms, improve the patient’s comfort level, pain management, educate the patient, gain patient’s compliance to the prescribed plan of care and assist the patient and family through end-of-life issues. The nurse is proficient in providing palliative and hospice care. Additionally, the nurse communicates with the registered nurse and members of the care team to insure proper coordination of care.
