The Coordinating Center is seeking nurses (RN) and licensed social workers (LMSW, LCSW-C) to provide care coordination services with our clients from your own home! The caseload will consist of all ages, with a focus on pediatric clients that have complex medical needs. You will be connecting the clients and their families with services and supports in the community setting. As a teleworking organization, we provide the flexibility to work from home while giving you the support you need to manage the needs of your clients. And best of all-- no evenings or weekends are required in this position!

Our social workers are called "Clinical Care Coordinators" and provide these services for our clients in our REM Program:

• Developing individualized assessments and plans of care


• Providing direct community-based resources for clients based on their individual needs


• Community + virtual visits with clients


• Telephonic support and monitoring of services


• Short-term and long-term follow-ups with clients in the community

Our Clinical Care Coordinators visit clients throughout their surrounding county/region to support their needs in the community, with mileage reimbursement available. We work with clients of all ages and have a collaborative approach to training, mentoring, and professional development.
